Fees & Payment
As of July 2011 my fees are as follows:
Initial evaluation $360 (60 minutes) - $540 (90 minutes)
Home or Facility Evaluation: $630
Psychotherapy (50 min): $240
Psychopharmacological Care (15-30 min): $180
Skilled Nursing Home/Assisted Living follow-up visits: $250
Telephone/Email Consultation (extended >15min): $180
Travel to homes or facilities beyond Central Marin is billed in 15 minute increments @ $360/hour
Payment is due at the time of service unless other arrangements have been made. I accept VISA/Mastercard, checks or cash.
I do not bill third party payors such as insurance companies or government programs (Medicare or MediCal). I will provide you with a statement that you may submit to your insurer for out of network reimbursement. Statements include all relevant information required for reimbursement including my tax ID number, ICD-10 diagnostic code and CPT procedure code.
